These Forms are to be completed by the Minister, 2 Ministerial Colleagues, 3 Local Church Leaders, the Conference President and the Ministerial Secretary. If you are unsure please leave the Score blank. Please note that the Minister has waived the right to review your submission which will be received anonymously by the Ministerial Secretary. Scoring system:

Carefully Consider and Score each question up to 10 stars:

1 STAR = NEVER/ VERY POOR

5 STARS = SOMETIMES / OK

10 STARS = ALWAYS / PROFICIENT

For a Minister to Pass their Current Level in each Marker, the following Scores are required:

LEVEL 1 Pass if average score is above 5.0

LEVEL 2 Pass if average score is above 6.0

LEVEL 3 Pass if average score is above 7.0

LEVEL 4 Pass if average score is above 8.0
